                                 (Only government issued ID's will be ACCEPTED.)

                                 (You may cover the first 12 digits of the card for protection)

     1. If cardholder is different from recipient, two ID's of recipient would also be required.



     2. Failure to submit these documents will void the transaction and payment will be reverted to the credit card owner.



     3. Rest assured that these documents will be for verification purposes only and will be kept private and confidential.
